We propose a methodology for digitally fusing diagnostic decisions made by multiple medical experts in order to improve accuracy of diagnosis. Toward this goal, we report an experimental study involving nine experts, where each one was given more than 8,000 digital microscopic images of individual human red blood cells and asked to identify malaria infected cells. The results of this experiment reveal that even highly trained medical experts are not always self-consistent in their diagnostic decisions and that there exists a fair level of disagreement among experts, even for binary decisions (i.e., infected vs. uninfected). To tackle this general medical diagnosis problem, we propose a probabilistic algorithm to fuse the decisions made by trained medical experts to robustly achieve higher levels of accuracy when compared to individual experts making such decisions. By modelling the decisions of experts as a three component mixture model and solving for the underlying parameters using the Expectation Maximisation algorithm, we demonstrate the efficacy of our approach which significantly improves the overall diagnostic accuracy of malaria infected cells. Additionally, we present a mathematical framework for performing ‘slide-level’ diagnosis by using individual ‘cell-level’ diagnosis data, shedding more light on the statistical rules that should govern the routine practice in examination of e.g., thin blood smear samples. This framework could be generalized for various other tele-pathology needs, and can be used by trained experts within an efficient tele-medicine platform. 相似文献

The effect of a two-vessel forebrain ischemia (induced by occlusion of carotid arteries and hypotension), subsequent reperfusion, and administration of indomethacin and quinacrine on the Na+,K+-ATPase activity and diene conjugate content was studied in various rat forebrain fields. The most pronounced metabolic alterations were observed during ischemia and reperfusion. Under these effects, there was a statistically significant reduction of the Na+,K+-ATPase activity in the brain cortex and striatum and an increase of the diene conjugate content in the rat brain cortex in comparison with sham-operated animals. Injection of indomethacin, a cyclooxygenase inhibitor, to rats subjected to ischemia and reperfusion, resulted to a statistically significant increase of the Na+,K+-ATPase activity in the brain cortex, hippocampus, and striatum (p < 0.02) as compared with control animals. The diene conjugate content in the rat brain cortex during brain ischemia and reperfusion was statistically significantly lower in the rats injected with indomethacin. The effect of quinacrine (a blocker of phospholipase A2) was similar to that of indomethacin in the rat cortex, whereas in the rat striatum and hippocampus, the quinacrine effect during ischemia and reperfusion was less marked than that of indomethacin. The obtained data indicate the ability of inhibitors of the arachidonic pathway of free radical formation to normalize the Na+, K+-ATPase activity during brain ischemia. The review addresses varied aspects of physiological and biochemical mechanisms aimed at creating special rheological conditions for blood flow termed non-Newtonian blood properties. We conducted a comparative analysis of structural features and phospholipid repertoire of the erythrocyte plasma membranes and cytoskeleton, extracellular ATP pool, and ecto-ATPase enzymatic activity in nucleated and non-nucleated erythrocytes in vertebrates, as well as a study of thermal effects in nucleated red blood cells. Based on data from the literature and our own research, we hypothesize that the phenomenon of non-Newtonian blood properties is underlain by a decrease in the relative blood viscosity due to thermal hydrolysis of extracellular ATP that erythrocytes release onto their surface most actively under capillary deformation stress. We believe that in fishes an important role in this process may belong to erythrocyte plasma membrane ecto-ATPases. Due to a heat released during hydrolysis of extracellular ATP, the marginal blood plasma layer, adjoining the capillary wall, appears to warm up. This may modify the structure of the membrane bilayer and deform the cytoskeleton, thus providing special rheological conditions for blood flow. The heat-producing ability, that we found in fish nucleated erythrocytes, may serve an additional evidence for the existence of this mechanism. 相似文献

The spindle checkpoint is a cell cycle surveillance system that ensures the fidelity of chromosome segregation. In mitosis, it elicits the “wait anaphase” signal to inhibit the anaphase-promoting complex or cyclosome until all chromosomes achieve bipolar microtubule attachment and align at the metaphase plate. Because a single kinetochore unattached to microtubules activates the checkpoint, the wait anaphase signal is thought to be generated by this kinetochore and is then amplified and distributed throughout the cell to inhibit the anaphase-promoting complex/cyclosome. Several spindle checkpoint kinases participate in the generation and amplification of this signal. Recent studies have begun to reveal the activation mechanisms of these checkpoint kinases. Increasing evidence also indicates that the checkpoint kinases not only help to generate the wait anaphase signal but also actively correct kinetochore-microtubule attachment defects. 相似文献

Serum levels of immunoglobulins G, A, M, and E (IgG, IgA, IgM, and IgE, respectively); allergenspecific IgE antibodies; and interleukin-4 (IL-4) were measured in ten cosmonauts before and after flights of 7–11 days on the International Space Station. Continuous wavelet analysis can be used to quantitatively describe local changes in cardiac rhythm oscillations. The following new indices of the wave structure of the cardiac rhythm were suggested: the rate of changes in the frequency of the oscillatory component within a given time interval, the lengths of the fading periods for specific frequency oscillations detected on the cardiointervalogram, and the changes in the instantaneous frequency-amplitude ratios during the observation period. A comparative analysis of the time and amplitude characteristics of the negative N200 and positive P300 components of visual evoked potentials recorded at symmetric points of the frontal, parietal, temporal, and occipital areas of the right and left hemispheres of the cerebral cortex has been performed in subjects with or without the skill of operating a computer. Subjects inexperienced in an operator’s work exhibited an interhemispheric difference in the time and amplitude characteristics of the studied components. In subjects that had the skill of operating a computer, the interhemispheric difference was little, which suggests that the cortex plays only a small role in the cerebral control of this activity. 相似文献
